The elderly driver involved in a fatal crash in which his wife was killed had previously had his driver's license revoked, according to records from the Registry of Motor Vehicles.

Katherine Casey, 84, of Malden, Mass., was passenger in a car driven by her 83-year-old husband, Joseph. She died after their car collided with another car in Woburn on Monday.

Joseph Casey, according the state records, had his licensed revoked as an "immediate threat" because of a medical condition in March 2004. It was reinstated by the Registry two months later.

Casey was at the wheel when their car crashed into another car driven by Claire Regan, 77, at the intersection of Russell Street and Cambridge Road.

Regan, who was taken to Massachusetts General Hospital, was seriously injured in the crash.

Casey had two accidents for which he was found at fault after his license was reinstated. In July 2004, he was involved in an accident in Woburn. Eighteen months later he was involved in an accident in Malden.

The Registry revoked Casey's license Tuesday as a result of Monday's fatal crash. A police investigation will determine if Casey will face criminal charges.
