Decoding Mammography Violations
The Massachusetts Department of Public Health performs an annual inspection of every mammography facility in the state. Team 5 Investigates created this database so you can view the inspection reports since 2004 for your facility.Level 1 Violations are "serious" and include facilities that operate without a license, use a radiological technologist who is not specially trained in mammography, and fail to notify the state within 30 days that the Responsible Physician changed. In addition, repeat Level 2 violations can become Level 1 violations.Level 2 Violations are "moderate problems" and include facilities that fail to perform quality control tests at the required frequency, employ staff who don't have enough continuing education credits, and fail to evaluate the mammographer during the first three months of employment.
Level 3 Violations are "minor" and include facilities that fail to record how often they cleaned the darkroom.In 2004, the state transitioned away from less specific terms like "substantially compliant" to describe Level 2 and Level 3 violations, and "violation" to describe Level 1 violations.
