"Please, lets just chalk this up to a tragic freak event, and maybe see if there can be some sort of guard added in to prevent this. If it turns out that there is a device that was not used, or was not used properly, or the hose just was not put back right, than this woman's life was taken because of them, and they should be held responsible."
